Nursing Shortages: Addressing it in Texas State, Houston Area, Medical-Surgical Specialty Area
Nurse have a direct impact on patient as they are the first person patients see. Nurses provide patient assessments, evaluations and care. During hospital stay, they offer most bedside patient care. They ensure care is delivered safely and effectively. Thus their shortage jeopardizes delivery of care that is safe and effective. Texas is facing a significant shortage of nurses it needs to protect the safety, health and welfare of its citizens. This concern is more pronounced in the Houston region where the demand exceeds the supply. This critical shortage has created difficulties for delivery of safe healthcare services. This paper explores strategies that can be used to address this shortage.
There is need to expand opportunities to allow nurses to fully engage with other health practitioners in redesigning the care. The profession of nursing has the potential to implement wide-reaching changes across many health settings (Whitney, 2018). Because of their close proximity to patients as well as their scientific understanding of care processes across continuum of care, nurses have a great opportunity to collaboratively work with other health professionals, to lead in redesign and improvement of the healthcare system (Boivin, 2017). The IOM report recommends the transformation of nursing education to prepare nurses who are capable of working collaboratively with other healthcare providers in in diverse settings (Whitney, 2018).
The state should allow nurses to practice to the full extent of their training. To ensure residents in Texas have access to required healthcare services and unique contributions of nurses maximized, state actions are needed to update scope-of-practice regulations to capitalize on the full capacity of advanced practice nurses (American Association of College of Nursing, 2020). Removal of institutional and regulatory barriers should be encouraged in order for the healthcare system to reap the full benefits of nurses’ skills, knowledge and experience in patient care. Advanced practice nurses should be allowed to expand their potential as primary care provides based on their education and training. Time is ripe for the state to remove these scope-of practice obstacles. Insurance companies should follow suit with financial and policy changes that provide patients with the freedom to choose from a variety of providers to best satisfy their health needs.
There is need to transform and improve nursing education to better prepare nurses for sweeping changes in the healthcare system. These fundamental changes will lead to improved patient outcomes. While the number of nurses enrolling in Bacclaurete and advanced levels has grown tremendously, there is shortage of nurse faculty and clinical placements to accommodate such numbers. Shortages of nurse faculty and clinical placements limited the opportunities available for clinical learning (Whitney, 2018). State board of nursing and accrediting bodies have done a good job to implement and support nurse residency programs. Nurse residency programs can speed up preparedness to fill needed nurse roles.
Medical facilities and health organizations should invest in professional development and long term training for nurses and encourage nurses to engage in lifelong learning. Nurses who participate in lifelong learning can gain competencies necessary to provide patient care to dive patient populations (American Association of College of Nursing, 2019). The IOM report recommends institutions to commit to continued professional development of nurses. Nurses who are taking up managerial and innovative skills require skills and knowledge and employers can take the initiative to train their nurses through onsite classes, distance learning, seminars, self-tutorials. In addition, nurses should be prepared and encouraged to take up leadership positions.
