Planning is an important aspect in any evidence-based practice (EBP) change project. It helps to ensure that the project is successfully implemented (DiCenso et al., 2016; Ellis, 2016). The development of individual success plan (ISP) requires the input of both the course preceptor and the learners’ program department (Hardey & Mulhall, 2018). This helps to ensure that the student develops an effective plan that can guide the implementation of his or her project (Kerzner, 2017). The ISP below is meant for the implementation of the proposed EBP project on effective disease control and prevention initiatives for marginalized groups. Table 1 shows a list of the proposed capstone project topics, indirect clinical practice experience assignments, and graded assignments. Table 2 depicts a personal plan based on direct clinical practice experience, course assignments and objectives, and aspects of self-assessment.
Individual Success Plan
Required Practice Hours: 100 hours for direct clinical experience (50 leadership hours + 50 community hours), and 25 hours for indirect clinical experience.
